Het gaat na mijn lensproblemen met het linkeroog nog steeds matig met mijn ogen. Lenstolerantie links is verminderd waardoor ik niet altijd kan doen wat ik wil. Gelukkig lijkt het wel over het algemeen voldoende te zijn om een redelijk normale dag vol te maken.

Omdat het UMC wat traag was met het inplannen van diverse onderzoeken en het ondernemen van actie, vooral wat betreft mijn rechteroog is er een brief naar ze uitgegaan met de vraag het allemaal een stuk serieuzer te nemen aangezien mijn praktische zicht achteruit lijkt te lopen en er sneller actie moet worden ondernomen.

Dat resulteerde vorige week in een consult bij Dr. Tahzib en Dr. van der Lelij van het UMC. Zij keken nog eens goed naar het rechteroogoog en hebben er ook voor gezorgd dat onderzoeken die voor eind februari stonden vorige week vrijdag zijn uitgevoerd.

Het doel van de onderzoeken was te kijken of mijn rechteroog schade heeft opgelopen van de verhoogde oogdruk die ik had en of daar maatregelen tegen genomen moeten worden. Ik zit namelijk op de maximale glaucoombehandeling en dat betekent dat er bij verdere verhoging van de oogdruk er geen verdere behandeling mogelijk is. De medicatie zorgt wel momenteel voor een continu lage oogdruk waardoor mijn rechteroog over het algemeen beter was dan eerst.

Deze onderzoeken zijn inmiddels uitgevoerd. Het ging om een dagcurve waarbij de oogdruk over de dag wordt, een OCT scan waarbij de dikte van de oogzenuw wordt vastgesteld een een gezichtsveldonderzoek.

Afgelopen dinsdag kreeg ik hier de resultaten van. Helaas is er vastgesteld dat mijn oogzenuw zwakke punten vertoont en dat een deel van mijn gezichtsveld minder gevoelig is geworden. Ik heb dus door de verhoogde oogdruk glaucoom opgelopen, zij het wel in een geringe mate.

Komende tijd zal de aandacht dus liggen op het voorkomen van verdere schade aan mijn oog. Hiervoor zijn de mogelijkheden bij glaucoom echter zeer uitgebreid. Zeker omdat het vroeg is vastgesteld en de oogdruk momenteel laag is verwacht ik geen verdere ontwikkeling van dit probleem.

Er is besloten om mijn oog te opereren een zgn. Baerveldt implantaat te plaatsen. Dit is een buisje dat in de voorste oogkamer achter het hoorvlies uitkomt en is verbonden met een “plaat” welke aan de oogbol wordt bevestigd. Door het buisje wordt overtollig oogvocht welke te hoge oogdruk veroorzaakt afgevoerd.

Resultaar van de behandeling is dat de oogdruk permanent met minder of geen medicijnen laag blijft en dat nieuwe transplanties mogelijk blijven omdat het oog beter onstekingsremmers (welke de oogdruk chronisch kunnen verhogen) kan verdragen.

De operatie zal in de komende maanden plaatsvinden, op 8 februari heb ik een preoperatief onderzoek want deze operatie vindt helaas (of niet?) plaats onder algehele verdoving.

Uiteraard zal ik proberen om na de operatie mijn ervaringen verder op deze website te omschrijven.

English summary:

Last week I had an appointment with two ophthalmologists at the Utrecht University Medical Center to examine my eye again. A letter was sent to them because further examinations were planned a bit too far in the future in my opinion.

Last week I also had the tests that were originally planned for the end of February. Unfortunately these tests have shown that my right eye shows signs of glaucoma due to the period of high intraocular pressure I experienced. A portion of my right eye’s vision has weakened in sensivity to light and the optic nerve shows some weak areas.

Luckily this all was detected very early and the glaucoma is very slight. It has been decided that a glaucoma drainage device (Baerveldt) is needed to permantently lower the intraocular pressure and reduce the need for medication as I am now on the maximum amount of meds to lower the eye pressure. If it ever rises again there is no way of preventing additional damage to the optic nerve.

Surgery will be performed in the following months. I don’t know what the impact will be but if I am able I will try to update this website about my experiences.

Just some venting,

Why on earth is every interesting technical discussion forum on Yahoo Groups? I absolutely loathe its design. When requesting the main forum it presents you with unthreaded, ungrouped messages by date. To group messages by thread you have to click a link, and even then you have to click each message separately just to view it. It’s 2012 damnit, stupid threaded message boards should be a thing of the past!

It absolutely puzzles me why certain technical stuff ends up on Yahoo Groups. Especially hamradio and geiger counter stuff seems to end up on Yahoo Groups by default. Maybe people setting up these groups favor the usenet style interface because they are somewhat older than me and continue to use the annoying, threaded usenet-style interface because that is what they initially came in contact with when first using the net. Don’t get me started on search results…

(BTW, I found a modern message board focused on geiger counters and all kinds of radiation detection stuff, guess what, it’s only in Italian!)

Another annoyance of me lately has been expensive USB to TTL serial adapters. Most are based on the well known FTDI chip which is used on Arduino boards. Simple adapters using this chip have prices ranging in the €/$10-25 range, just for a simple device presenting a serial port to the host operating system and outputting TTL levels.
I guess the CEO of FTDI gets a nice bonus each year, selling loads of these chips for $3 when they likely cost pennies to make.

I’m still looking into a decent alternative, €15 TTL adapters suck!

Tranentest / Tear testing @ Oogziekenhuis Rotterdam

English summary below

Afgelopen week ben ik vanwege de matige toestand van mijn getransplanteerde oog langs het Oogziekenhuis in Rotterdam geweest. Uiteraard heb ik ook gelijk de problemen met mijn linkeroog aangekaart.

Het was een productief bezoek want ondanks de drukte en het lang wachten op de arts en overleg met andere artsen werd er een concreet oordeel gegeven. Mijn rechteroog moet op termijn een nieuwe transplantatie ondergaan omdat de toestand van het transplantaat slechter is geworden en dat hier met medicijnen weinig aan te doen is. Er zal vanwege de hoge oogdruk wel door een glaucoomspecialist naar het oog gekeken moeten worden om te kijken of de oogdruk operatief (dmv een drain) tijdelijk verlaagd kan worden. Na de hertransplantatie moeten namelijk dezelfde medicijnen worden gebruikt waar ik nu al slecht op reageer met een te hoge oogdruk.

Wat betreft mijn linkeroog dacht men in het OZR aan crosslinking om hopelijk de slechte lenstolerantie van de laatste tijd mee te verbeteren en een transplantatie hopelijk voorlopig te kunnen vermijden. Men zag eigenlijk niks van de droogheid die eerder in het UMC werd gezien en vond dat de lens weer in kon. Het lijkt erop dat de problemen meer door slechte lenstolerantie dan door droogheid komen.

Het OZR heeft dit alles samengevat in een brief welke richting het UMC gaat.

Ik ben de dag erna naar het UMC geweest waar al bekend was dat ik in het OZR was geweest, de arts in het UMC en die in het OZR hebben namelijk regelmatig mailcontact. De brief was uiteraard nog niet binnen maar de arts die de supervisie heeft gaf wel aan dat de inhoud van de brief te verwachten was. Ze heeft voor het einde van het jaar een afspraak ingepland om een glaucoomspecialist naar het oog te laten kijken aangezien de druk van het oog eerst omlaag moet voordat er operaties plaatsvinden.

Ik ben aan de ene kant blij dat er nu wat schot in zit maar aan de andere kant maak ik me zorgen over het linkeroog waar ik afhankelijk ben voor mijn werk en eigenlijk alles wat een beetje normaal zicht vereist.

Last week I visited the Rotterdam Eye Hospital for a second opinion on the status of my right, transplanted eye which has become increasingly problematic. The doctors in Rotterdam argued that my right eye needs a retransplant because the current transplanted cornea will likely not become better. First however, the eye pressure has to drop because a new transplant requires large doses of medicine which increase eye pressure even further. They suggested that first a glaucoma specialist has to examine the eye and see if it’s posssible to surgically lower the eye pressure by temporalily implanting a drainage device.

Of course they also looked at my left eye which has had problems with dryness and low lens tolerance. They said that corneal crosslinking was an option consider to delay or postpone a corneal transplantation. Also, if a transplant was needed they suggested that the UMC looks into the possibility of doing a partial transplantation to enable a quicker recovery and lower chance of complications.

The doctors sent a letter to the Utrecht University Hospital with their findings.

The next day I visited the UMC and talked to the doctor who supervises my corneal transplant. She hadn’t received the letter yet of course, but agreed on the contents that I described to her. She has made an appointment before the end of the year to have a glaucoma specialist examine my right eye and look into (surgical) options to lower the eye pressure.

So, in the end, a little vigilance paid off, I got other doctors to look at my eyes and suggest that action has to be taken to improve my vision. On the other hand I am worried about my left eye, if lens tolerance stays low this will have a continuous impact on my life.

In the picture you can see the HV generator on the top of the board, on the lower left the Arduino standalone parts along with a MAX232 as a level converter for easy serial hookup and on the right two tubes connected in parallel, a SI-29BG (beta and gamma) and a SI-12B for added alpha sensivity. Also shown are the USB>Serial adapter and a separate USB connector to provide 5V to the board.

I have had it running for some weeks with a Russian SI-29BG hard beta and gamma tube. These are nice, very sturdy tubes to experiment with. They easily detect natural background radiation and K-40 from a bag of Potassium Chloride.As I already explained I have connected two tubes in parallel. There wasn’t that much variation in background radiation, (see below) even after heavy rainfall so I am now going to watch whether alpha sensivity makes any difference.

Also, the SI-12B is kind of useless on its own, it is not very sensitive to beta and gamma but is able to detect alpha quite well due to its mica window.

Scripting

I have also changed my scripting on both Linux and Arduino to make everything easier to test and make it fail gracefully when the counter stops functioning for some reason. It also now works in Cacti with a GAUGE type of data, not a counter. This prevents spikes when the counter is reset or temporarily disconnected or offline. The script is as follows:

I have put everything in one script and created functions to log the counts over  the last 5 minutes or display the last logged 5 minute count. If the logfile wasn’t updated properly it will return an error, causing Cacti to not record a value and show the error in its logfile. The script is connected to SNMP as described in my original article.

Basically, it counts the pulses, outputs the last five minute count every 10 seconds using the serial port and clears the counter after 5 minutes preventing any overflows of the unsigned long value (very unlikely). It also gives a very short tone with each interrupt which results in a little clicking sound. The random clicking helps me get to sleep quicker .

Well, you could say that’s the free market in action. But it wouldn’t surprise me if prices are driven up by people not actually needing geiger counters but buying them anyway out of fear while they are not likely to experience any effects of the disaster.

Today I saw something much worse, some German on Ebay trying to sell a “geiger counter” which actually is a high range survey meter, a CDV715 according to the picture.

This person is even recommending this device to Japanese people. I would say that is a very bad recommendation because:

• this meter is high range, if the needle moves you are likely in the exclusion zone around the reactors where one shouldn’t be anyway.
• it will not measure smaller amounts of radiation, you cannot use it to check whether food or water has been contaminated or not, or compare contamination levels.
• it will mostly detect gamma radiation, so it won’t properly detect the beta radiation coming from the Iodine-131 release from the reactors which is something you really want to be able to measure.

It kind of sickens me that there are people trying to make money of this disaster while thousands of Japanese died and more are homeless, and have to cope with possible effects of fallout from the leaking reactors.

Finally, €199 is way overpriced for a survey meter, I bought a German made one for a mere €10. An original Civil Defense CDV715 should cost €100 at most.

It’s also possible to use cheaper, easier to find tubes with the CDV-700.  I found the following post from radio amateur K0FF on the “Geiger Counter Enthousiasts”  group on Yahoo groups. It describes a simple circuit to adapt tubes of varying voltage to the CDV-700. I built one, and it works!

I created a schematic to help people build this very handy circuit:

Parts:

R1 = 3.3 megaohms resistor 0.25 watts
R2= 1.8 megaohms resistor 0.25 watts
C1 = 10 nanofarad capacitor, rated 2 kilovolts
D1 – D3 = any number of zener diodes, the values added together  should be the desired voltage to regulate to

You can build this circuit in any way you want, keep in mind that there is high voltage involved, make sure that everything is properly insulated and that bare conductors are not closer to eachother than 2mm to prevent any arcing.

This circuit is basically a voltage regulator such as a 7805 built with discrete components and for higher voltages. To regulate to 500V for example, I used one 200V and two 150 zener diodes. 500V is a common voltage used by GM tubes.

With this circuit you are no longer limited to the more rare 900V tubes and you can connect all kinds of tubes to the CDV-700.  Keep in mind, however, that the CDV-700 may no longer display correct measurements in milliRöntgens per hour since the scale is calibrated for use with the 6993 tube.

Afgelopen vrijdag werd ik wakker met een vuurrood oog nadat ik de vorige avond al wat irritatie had. Kijken naar lichtbronnen gaf ook minder prettige pijnsteken in het oog. Zoals het hoort maar gelijk met een UMC gebeld waar ik dezelfde ochtend nog langsging. Daar stelde men vast dat er tekenen van afstoting te zien waren.

Zoals gebruikelijk in een academisch ziekenhuis keken er eerst relatief onervaren mensen naar het oog waarna er mensen met meer ervaring naar gingen kijken, tot aan een arts van het cornea-team toe die mij vertelde dat het goed was dat ik zo snel met het ziekenhuis had gebeld.

Om de eventuele afstoting te voorkomen moet ik nu 6x per dag druppelen met Dexamethason en ‘s nachts een antibacteriële oogzalf gebruiken.

De dag erna was het oog gelukkig al een stuk minder rood en waren de pijnsteken bij licht vrijwel weg, al heb ik wel iets meer lichtgevoeligheid in het algemeen. Mijn basiszicht zonder lens is ook teruggelopen, het is wat mistiger en onscherper.

Het vervelendste is natuurlijk dat ik voorlopig mijn goede zicht in mijn rechteroog waar ik zo blij mee was moet missen. Het is weer een kwestie van zowel thuis als op werk de resolutie van het computerscherm omlaag zetten. Hopelijk herstelt het oog zich weer snel en kan ik de lens weer in.

Update 20/4:  2x langs het ziekenhuis geweest voor controle, eerst maandag voor een algemene controle. Oogarts stelde vast dat de losse cellen in de voorste oogkamer waren afgenomen dus dat alles er iets beter uitzag.

Ik ben vanwege klachten dinsdag weer langsgeweest, ik had een stekend gevoel en 2 “putjes” in het hoornvlies. Het oog bleek er echter verder goed uit te zien. De klachten leken te komen door droge plekjes welke worden veroorzaakt door het oedeem (vochtophoping) aan de kant van mij neus, een plek welke er al langer zat en welke dus nu problemen lijkt te veroorzaken. Medicatie zal worden aangepast om de oogdruk verder omlaag te krijgen om te kijken of dat het oedeem vermindert. Beetje vreemde actie aangezien die plek er al maanden zat, maar in principe snap ik het wel aangezien er nu problemen door die plek komen.

Het gaat dus beter met het oog en afstotingsverschijnselen zijn er niet meer. Wel ben ik redelijk vermoeid door het moeten kijken met 1 oog. Lens kan waarschijnlijk pas weer over 1,5 week in…

English summary:

Last friday I woke up with a painful bloodshot eye. I of course immediately called the hospital for an emergency checkup. In the hospital signs of rejection were seen in the eye, so I had to take additional steroid drops six times a day and an antibacterial ointment for the night.

I experienced pain when looking at strong light sources but that seemed to be over the next day. The eye looks a lot better but still feels a bit irritated and my base sight without the lens is decreased, its a bit more hazy and blurred than usual.

I hope the treatment works because in the meantime I cannot wear a lens so I have to make do with the vision in my left eye which is sufficient but nothing more than that.

Update 20/4:

Last monday I had a checkup, eye looked fine, signs of rejection diminishing. On Tuesday I had some pain in the eye and two “spots” so I went to the hospital a day earlier than the planned checkup.

Basically, everything is going better, no signs of rejection anymore, but what was found were dry spots likely caused by swelling of the cornea in the part of my eye that always had some oedema (fluid buildup). I have to use different medication to reduce eye pressure further to see if the oedema (which has been present for months but didn’t cause problems) can be reduced.

Generally I am less worried but tired of having only my left eye to see with, hopefully I can wear the lens again, doctors say it should be possible in one and a half week.

Vandaag was ik weer voor controle in het UMC. Er werd eerst door een assistent van de oogarts gekeken hoeveel zicht er uit het oog was te halen. Eerder keek men alleen naar het ongecorrigeerde zicht, maar deze keer ging men kijken hoeveel zicht er met correctie was te behalen. Dat was dus een visus van 0,60.  Helemaal niet slecht dus. Het verbaasde me hoe scherp zicht ik van de behandelkamer had kijkende door de phoropter.  Om dat zicht te geven moest er wel een cylinder van 8,5 worden toegepast. Ach, alles beter dan de oude situatie…

Daarna langs de oogarts voor de rest van de controle. Dr. Rouwen en een arts in opleiding keken naar mijn oog. Alles was volgens hem “rustig” maar de slecht genezende plek aan de kant van mijn neus zat er nog steeds. Dit was volgens hem echter geen reden tot zorg maar meer een kwestie van afwachten. Dr. Rouwen legde uit dat op die plek het lichaamseigen endotheel nog niet goed aansloot op het donorweefsel waardoor het vocht niet voldoende uit het hoornvlies wordt weggepompt. Dit leidt tot ondoorzichtigheid.

Verder was de oogdruk weer wat opgelopen tot de waarde van 21. Aangezien het oog er goed uitzag werd er besloten dat de Vexol oogdruppels van 5 naar 4x per dag konden. Corticosteroïden verhogen namelijk de oogdruk dus als het mogelijk is worden deze druppels verminderd. Een oogdruk van 21 is trouwens een vrij normale waarde.

Dr. Rouwen vond ook dat er wel geprobeerd mocht worden een lens aan te passen. Hier was ik erg blij mee want met een lens zou het zicht wel eens beter dan links kunnen worden en mijn totale zicht sterk verbeteren! In de komende dagen ga ik een afspraak maken met mijn lenzenspecialist om te kijken wat er mogelijk is.

Als laatste, in december zullen de helft van de hechtingen worden verwijderd. Dit is een vrij kleine ingreep die onder plaatselijke verdoving plaatsvindt. Daarna zal ik 1 dag met een verband rondlopen en na 2 dagen zou het genezen moeten zijn. Lijkt me verder geen superschokkende gebeurtenis.

Uiteindelijk lijkt het dus allemaal toch wel de goede kant op te gaan…

And now the same story, maybe a little bit more shorter, in English:

Today I visited my ophthalmologist for a checkup on my right eye. First my eyesight was checked by an assistant. Using the phoropter it was possible to achieve a visus of 0,60 which was quite good vision compared to the uncorrected vision of the eye. I was quite astonished how much I could see of the room through the apparatus.

After the eyesight examination my eye was looked at by Dr. Rouwen and someone studying to be a doctor. My eye looked fine, except for a slow-healing patch on the left side (from my perspective)  of the cornea. My doctor told me that this area wasn’t properly covered by the endothelium yet which causes a build-up of fluids in that part of the cornea. If corneal tissue has to much water in it, it will become cloudy. He wasn’t completely satisfied with the healing on the left side but said that it was no cause for worry.

I was also told that my orthoptist can now try to fit a lens on the eye. This was nice to hear, because with a lens the corrected vision will likely be more than 0.60, probably more than my left eye (current visus 0.75). So, one of these days I will contact my orthoptist to make an appointment.

Also, in december my doctor will remove half of the stitches. This will be done under local anaesthesia and afterwards the eye needs to heal for 2 days. No big deal compared to the actual surgery I assume.

Anyway, this was my latest update on KC and the corneal transplant of my right eye.

Mijn lenzenspecialist vond laatst dat mijn geopereerde oog nogal droog was. Hij zag met de spleetlamp droge “spikkeltjes” op het hoornvlies en raadde mij aan mijn oogarts er iets voor te vragen.
Ik wilde dat voor de komende controle aangepakt hebben dus een telefonisch consult aangevraagd wat ik dus gister had. De oogarts erkende dat droge ogen veel voorkomen bij hoornvliestransplantaties.

Bij een hoornvliestransplantatie worden de zenuwen naar het oude weefsel doorgesneden. Omdat het nieuwe weefsel eerst geen verbinding heeft met het zenuwstelsel van het lichaam zal het geen gevoel hebben. Dat is waarschijnlijk ook de reden dat de operatie weinig pijn deed. De pijn is er misschien wel maar kan simpelweg niet je hersenen bereiken. Uiteindelijk zullen de zenuwen in het nieuwe weefsel groeien en het wel gevoel geven.
Omdat het oog in het midden niks voelt zal het maar gedeeltelijk voelen dat het oog droog wordt. Het oog maakt dan minder traanvocht aan. Voor dit probleem heeft de oogarts het middel Vidisic voorgeschreven. Dit is een kleverig goedje wat het oog lang vochtig houdt, beter dan bijv. simpele fysiologische zoutloplossing.

Ik merkte trouwens zelf vrij weinig van de droogheid, behalve dan dat ik mijn rechterooglid een beetje voelde “slepen”. Last had ik er verder niet van.

During my last visit to my lens specialist he noticed that my transplanted eye was quite dry. Using the split lamp he saw small “speckles” on the cornea due to dryness. He suggested to talk to my ophthalmologist about it, so I arranged a consultation over the phone, since my next appointment is not that far in the future, and this seemed to be a small issue.

He explained to me that the eye becomes dry quite often after corneal transplantation. This is because the transplanted tissue is not able to signal to the body that it is becoming dry, because the nerves responsible for carrying these signals are not attached to the cornea anymore. However, in the course of several months the nerves should reattach, returning feeling to the transplanted part of the eye.

To treat the dryness, the doctor subscribed me Vidisic, a sticky eye gel that moistens the eye and works longer than just saline because it has added viscosity.

By the way, I didn’t have any feeling of dryness in my eye, but I felt some friction of the eyelids when blinking. Still, my orthoptist and ophthalmologist were right about the dryness.

Anyway, other than some very slight complications everything seems to move forward with my eye. Still impatiently waiting though, to get some proper vision out of the eye so that I can look with two eyes again…
